**Performance Lacrosse’s Camps and Clinics will use US Lacrosse insurance. If your child is not a member please sign up at www.uslacrosse.org. This only costs $25 for youth players and covers the child under any lacrosse related injury/accident for a year. Once you have filled out the form online and paid, US Lacrosse will issue you a member number. EVERY player MUST have a member number on their registration before they may signup.

I certify that my child is in good physical condition and is physically and psychologically prepared to participate in a camp program. I hereby give my consent to Performance Lacrosse to provide my child, through staff of its choice, customary medical/athletic training attention, transportation, and emergency medical services as warranted in the course of my child's participation.

I am fully aware of, and appreciate the risks, including the risk of catastrophic injury, paralysis, and even death, as well as other damages and losses associated with my child's participation in a camp event. I further agree on behalf of myself, my child and my heirs and personal representatives that the Performance Lacrosse, along with the coaches, volunteers, employees, agents, officers, and directors of this organization, shall not be liable for any injury, loss of life, or other loss or damage occurring as a result of my child's participation in the event.

Refund Policy: If a camper withdraws after registering, you may request a refund (minus a $30 processing fee) by sending an email at least one week before the starting day to info@performancelacrossetraining.com. This does not apply to private/small group instruction payments. No refunds will be issued after clinic has begun.

Weather Policy: Once a clinic/camp has begun there will not be a cancellation unless severe weather is present. If such an instant occurs that session will be cancelled and there will be NO make up session. Successive weeks of clinics/camps prevent us from moving sessions.
